I need a formula that will look across each row, and return a value from either of 2 columns based on information in another column.
In other words, fill in the "send paperwork to" column with Insurance4All for Jim; BigShield for Robert;Dr. Smith for Michael; etc.

Name Insurance Plan Physician Who updates paperwork? Send Paperwork to
Jim Insurance4All Dr. Jones Insurance Plan
Robert BigShield Dr. Jones Insurance Plan
Michael BlueCompany Dr. Smith Physician
Vincent Insurance4All Dr. Casper Physician
Richard Insurance4All Dr. Doe
Insurance Plan

Your data probably lost formatting so it is bit difficult to figure out but you need VLOOKUP if data is vertical.

You can upload a sample workbook on this site. So please post a sample workbook with your data layout and couple of manual examples to get good reply.
 
sorry. here's the sample file. since the data I'm looking for is not vertical, I looked at using HLookup, but is that the right approach? I'm trying to write the formula to auto-fill Column E. So, if Column D says "insurance plan" fill Column E with the data in Column B. Hope this clarifies my issue.
