How to handle multiple input that are needed for one cell. The multiple fields can not be empty if the other field has information that is need for the cell that holds the calculation.

I was watching a video in which a company had created a control in excel that would popup and allow you to enter multiple pieces of numeric information in a form field and return one value for the excel cell field that it popped up from. But not a calendar control. Does anyone have a sample or video on how to create a popup form with multiple fields that will return one value for the excel cell. I am not a UI/UX designer but I am looking for some good examples of how this can be done. I do not want to buy any special controls for this project.
